Bradley Creek Health Center, located on the 24-acre campus of Carolina Bay within Wilmington’s Autumn Hall mixed use community, offers a full range of health care services to meet the needs of the individual. From Inpatient and Outpatient Rehabilitation to Assisted Living, Memory Care, and Skilled Nursing Care, the staff of Bradley Creek provides services for not only the residents of Carolina Bay but also the surrounding Wilmington area. Our rehabilitation and skilled nursing center has spacious private rooms that include television, wifi, refrigerator, and large private bathrooms with walk in showers. All of which assist in making your stay with us as comfortable as possible.
