- SinusitisChildren are special, but we all knew that. The facial sinuses are not all present at birth and continue to grow into the teenage years. Pediatric sinusitis has a special presentation, often involving nighttime cough but no facial pressure or pain. Congestion is very common. Surgery can be considered in children (just as in adults) if medical management has failed. Allergy evaluation and treatment are very important for most children who present with sinus disease. Adenoid enlargement (behind the nose) is usually a significant contributor to pediatric sinus disease, and adenoidectomy might be the first surgical treatment option.
- Medical Weight LossReflux disease is a very common cause of sore throat. Most people have experienced GERD—gastroesophageal reflux disease. In our office, we more likely see LPRD—laryngopharyngeal reflux disease. This involves stomach contents (acid, pepsin, etc.) getting into the throat, usually at night. It causes sore throat, swallowing and voice changes, a lump in the throat (throat clearing, globus), and cough. Management usually involves diet modification, weight loss, raising the head of bed and sometimes medications. We usually look at the throat including the vocal cords with a small fiberoptic endoscope. This gives us a lot of information about vocal movement, lesions, nodules, and so on.
- SnoringPrimary snoring: Â Snoring typically is caused by vibration of tissues within the oral pharynx (the uvula and palate). Nasal obstruction can be another cause. In mild snoring, these measures may help: losing weight, sleeping on your side rather than your back, avoiding sedatives and alcohol. Surgery can reduce the size of the soft tissue of uvula and palate, and also relieve nasal obstruction. Dr. Janning performs several technologically advanced procedures to treat snoring and sleep apnea.
- Ear InfectionsEar infections are pediatric problems related to Eustachian Tube (ET) dysfunction in the back of the nose. The ET is immature in children, and with recurrent head colds especially in winter months, the ears often get infected. Your child might have symptoms like poor sleep, ear pain, and fever. An exam of the ear drum will show pus in the middle ear. Treatment is with antibiotics, ibuprofen and acetaminophen. If your child has recurrent infections, hearing loss or complications of the infections, our professionals might suggest inserting ventilation tubes which will relieve the ET dysfunction for months to years. Ventilation tubes in children are done under general anesthesia and take only a few minutes with a quick recovery and instant improvement. The procedure can be done on adults as well.

- HypothyroidismSleepiness is often called hypersomnia or excessive daytime sleepiness. This is a very common disorder and can be devastating to your life style. There are many causes of sleepiness (most often insufficient sleep and sleep apnea). Other causes include medications, alcohol use, tobacco, metabolic disorders (hypothyroidism), anemia and many others. At Janning ENT Center, we are very excited to help patients with sleepiness because the improvement in lifestyle is so dramatic! Other than treating the cause of sleepiness, stimulants can be discussed and are very effective for most patients.
- ThyroidSnoring is palatal flutter. In some patients it also involves nasal congestion, but usually it is related to the back of the throat (soft palate, tongue). It is exacerbated by obesity, alcohol, back sleeping, sedatives and narcotics, thyroid disease and many other disorders. Simple snoring is a cosmetic disorder, and we are excited to help give options to treat this. The bed partner will be happy with the results! Other than weight loss, quitting smoking and side sleeping, we have performed several types of procedures on the throat and nose to improve snoring. A dental device (mandibular advancement device) can also be very effective.
- Thyroid CancerA thyroid nodule is very common, especially in women. This is usually evaluated in our office with laboratory tests, a needle biopsy, ultrasound and sometimes other testing. Nodules can be benign or malignant and may be removed. Surgery is called a thyroidectomy which is very successful in treatment of thyroid cancer. Any nodule noted in the lower neck should be evaluated in our office.
- UltrasoundSalivary gland swelling is fairly common. The major saliva glands are in front of the ear (parotid gland), just under the jaw (submandibular gland) and along the floor of mouth (sublingual gland). There are also many smaller glands throughout the mouth and throat that can be swollen or infected. Swelling of the glands is usually due to infection of the duct that carries saliva into the mouth. Infection is treated with antibiotics, good hydration, and lemon drops or lemon in water, which enhances saliva formation. A mass in any gland might need a biopsy and imaging (CT scan, ultrasound). Surgical excision is done for cancer and is usually very well tolerated and curative.
- X-Rays
- Computed TomographyThis newer technique is sometimes office based so you are not under general anesthesia. A small balloon is inserted into each sinus and inflated, remodeling the opening and offering long term resolution of sinus pressure and infections. The recovery is much faster after balloon sinuplasty surgery and bleeding and narcotic use for pain are reduced as well. Overall costs can be significantly reduced in many patients. Image guided sinus surgery (with CT scan) can often be performed. Image guidance allows accurate localization of the sinuses.
- ChemotherapyDr. Janning has extensive research and has published articles on various topics, with a primary interest in ototoxicity (hearing loss from chemotherapy drugs).
- Radiation TherapyAny mass in the neck should be evaluated in our office. At Janning ENT Center, we can help determine if the mass is concerning and what further work up and treatment are needed. Cancer in the neck (excluding thyroid cancer) is most often called squamous cell carcinoma. This is a very serious cancer and needs urgent management. The primary source of cancer is NOT from the neck but usually from a part of the aerodigestive tract. This could be the tonsil, base of tongue, vocal region, nasopharynx or other areas. We will strongly recommend further evaluation with a flexible fiberoptic endoscope to look at these structures. Imaging is needed, usually a CT scan. Biopsy is also needed and often can be done easily in our office. Squamous cell carcinoma is almost always secondary to tobacco use. It is imperative that the patient quit using tobacco. Surgery is often the treatment, and options will be discussed in our office. Other treatment like radiation therapy and chemotherapy (medications) will be considered. There are many other less common types of neck cancer. Please make an appointment with us for any concerning neck mass.

- OtolaryngologyDr. Mellema was born in Oklahoma and moved to Willmar at the age of 2. He has lived in this area his whole life. After studying at the University of Minnesota School of Medicine in Minneapolis, Dr. Mellema did his residency at the University of Cincinnati College of Medicine in Cincinnati for Otolaryngology, specializing in head and neck surgery. He has certifications with the American Board of Otolaryngology and the American Academy of Otolaryngology – Head and Neck Surgery.

These include medical and surgical management of sinus disease, facial (nose, ears, lips, eyelids, cheeks) cancer and lesion excision with reconstruction, management of hearing loss and ringing (tinnitus), and management of sleep disorders (sleep apnea, snoring, restless legs, insomnia, sleepiness, and others). He also enjoys treating blepharoplasty, rhinoplasty, otoplasty, coblation-assisted-tonsillectomies and thyroidectomies to help his patients feel their best.

- LesionsHoarseness or dysphonia is commonly caused by vocal abuse, infections (usually viral laryngitis), acid reflux, lesions and nodules, vocal cord weakness and other causes. It is important to have the vocal cords evaluated in our office if hoarseness has been present for a few weeks. This is especially true in smokers, who are at significantly higher risk for cancer. A small flexible fiberoptic endoscope is placed through the nostril to better evaluate the vocal structures. Treatment depends on the cause. A vocal polyp or lesion might need resection or biopsy. Most patients are instructed to hydrate well, use good voice hygiene (avoid yelling, coughing, whispering, singing, and so on) and see a voice therapist.
