It is always our effort to provide the highest quality medical care and encourage communication with your healthcare provider and now, Premier Family Health & Wellness is seeking national recognition as a Patient-Centered Medical Home. What does this mean to you? A Patient-Centered Medical Home supports a coordinated approach to patient care across all healthcare settings and places emphasis on providing medical treatment that is evidence-based and sensitive to patients’ cultural needs. What you need to know: 1. At Premier Family Health & Wellness a dedicated a team of professionals will coordinate your care. If your provider-of-choice is not available at your time of need, please ask to see his or her care-team partner. This may be a nurse practitioner or physician working closely with your provider. 2. Access to medical care beyond normal business hours is available through Premier’s Urgent Care Center open Monday through Friday from 8:00am to 8:00pm, and on Saturday from 9:00am to 4:00pm. At the Urgent Care Center, care is provided for same-day appointments as well as urgent care needs. 3. A Premier healthcare provider is on-call after hours for urgent matters. Please call 561-798-3030 and follow the prompts to reach the on-call provider. 4. You have access to a personal, secure Patient Portal. Here you can request appointments, referrals and medication refills. You can also access your health records, send an email to your provider and find self-help materials. Our staff can assist with activation once you provide us with an email address. 5. It is your responsibility to provide your medical provider with accurate health information. Make sure to notify us if you have been hospitalized, are seeking the care of specialty physicians, and about any medications and supplements you are taking.
