Community Health Network of Connecticut, also known as CHNCT, is a nonprofit health plan that provides health care services in Connecticut. It features more than 175,000 members and operates a network of primary and specialty care, inpatient, rehabilitation and outpatient locations. The organization also maintains contract agreements with other companies to provide transportation, pharmacy, dental and vision solutions. Community Health Network of Connecticut includes the Charter Oak Health Center, Fair Haven Community Health Center, Generations Family Health Center, Hill Health Center, Southwest Community Health Center and StayWell Health Center. It maintains a foundation that supports nonprofit activities of community health centers.
