      Lake Park is a more than 250-bed long-term skilled nursing care facility located in Sylvania, Ohio. It is a member of ProMedica Health System and is certified by the Ohio Department of Health to meet Medicare, skilled Medicaid and participation requirements. Its rehabilitation services include physical, speech, occupational, recreational and respiratory therapies. The organization s staff includes social workers experienced in geriatric care and a chaplain who provides guidance and counseling services. Other services provided by Lake Park include respite care, end-of-life palliative care through hospice unit, Alzheimer s and dementia care, gift shop, beauty salon and barbershop. Lake Park s nursing team works in coordination with nurses, therapists, physicians and social workers at the Flower Rehabilitation Center in Ohio.
