Feb. 6th, 2012 On Jan. 30th, 2012, I saw Dr. Mulpuri. When I got home I notice on the paper work you give me, it stated that my Primary Insureance was Health Choice. I do not think this is correct. Am on Medicare, which I do believe is my Primary Insurance. AHCCCS, Health Choice of AZ, would be the secondary. I could be wrong, but just incase.
