Hospice professionals of metro Atlanta is a Medicare approved hospice program. OUR team of professional hospice and palliative care has more than 25 years of combined hospice experience. OUR team includes physicians. Nurse practitioners. Registered nurses. Licensed practical nurses. Chaplain. Social workers. Licensed counselors. Licensed dietitian. Certified nursing assistants. Homemakers. And volunteers. We develop a plan of care that is unique to each Patient’s needs. ALL team members. Including volunteers. Are required to undergo criminal background checks. We commit ourselves to making each patient and their needs OUR priority. We provide care where YOU need it – AT home or in a facility.
