I am thankful that a Take Care Advisor registered nurse geriatric care manager was able to go with Mom to her doctor's appointment. As her daughter, and being out of town, I have never gotten much information from her about doctor visits beyond everything is fine or only if there was a medication change. I feel so much better knowing that in general her health is good. I like that the doctor addressed the items that your care manager requested her follow up with. They were able to help Mom plan a follow up visit which is not too far away. Interesting that Mom admitted to some memory problems. She really does fairly well, but I think she is easily overwhelmed with information. From this far a distance it has been very difficult to really know what is going on with Moms medical situation. The Medical Advocate Program (MAP) is taking away a lot of the worrying, wondering, assuming, hoping for the best, etc. that I been feeling. Thank you!
