Form & Function Physical Therapy is an elite level provider of sports injury rehabilitation and prevention services to athletes of all ages and abilities. We serve professional athletes and franchises, collegiate athletes, and adolescent and pre-adolescent athletes (ages 8 to 18), as well as individuals who play sports recreationally or simply like to exercise. We are a proud partner of the Ontario Fury Professional Soccer Club and serve as the club’s exclusive physical therapy provider. Our practice is led by Trevor Field, a trained physical therapist with a passion for helping patients see immediate improvement in their strength and performance after an injury. Trevor grew up in rural central Minnesota, enjoying all the sports available in the area. He continues to enjoy fishing and pitching for several baseball teams in Orange County with the MSBL, NABA Elite Wood Bat Leagues. He received his Bachelor’s degree from the University of Minnesota, and a Master of Physical Therapy degree from the College of St. Catherine. He has treated over 10,000 patients in his career, helping people suffering from pain and injury unlock their full healing potential. Form & Function Physical Therapy participates with many PPO insurance providers as an out of network provider. For patients without qualifying insurance, we offer affordable treatment packages, payment plans, and financing options.
