Memory Care Center A 59 bed secured unit for residents with Alzheimers and other related forms of dementia. Skilled Nursing Services We are staffed around the clock with licensed nurses providing skilled nursing care to our 120 bed facility. Guardian Care also operates a 42 apartment facility for fully functioning seniors, who are able to live completely on their own.

if you love your peoples, your elders... DO NOT! take them to GUARDIAN CARE NURSING HOME! I am very supportive of the black neighborhood and black owned businesses, but I refuse to support black businesses that take advantage of blacks(or anyone) who support black businesses! Your loveones dont deserve the treatment that they WILL receive from GUARDIAN CARE NURSING HOME 2500 W. CHURCH ST. The roaches are out of control, the customer service is unacceptable, the CNA's and RN's consistently sleep during the shift, and the only time they do what they are suppose to do is when they know that the state inspectors are coming! This is not anonymous, I stand by what is said in this review!!! Do your family deserve this?!?
