LifeCare Health Services serves all of East Texas as a Medicare-certified provider of home health and hospice services. With a service area extending 70 miles from both our Longview, TX and our Nacogdoches, TX offices, LifeCare delivers home health and hospice care to families in 30 counties of East Texas essentially covering all of East Texas. Founded in 1995 by the Wilcox family, LifeCare is still managed by two generations of the Wilcox family. From our family to yours, we are dedicated to providing healthcare that is caring, patient and competent. Our primary services include Hospice, Palliative Care, and Home Health. Through hospice services, LifeCare delivers in-home, healthcare services focusing on dignity and quality of life. LifeCare professionals ease symptoms associated with life-limiting illnesses. Most patients can attain an acceptable level of comfort through medications, counseling, and therapies. With LifeCare, your family doctor still directs your medical treatment, and we follow your instructions and the instructions of your doctor(s). Caring for a loved-one at the end of a long illness can be trying. Nights, in particular, can be lonely and frightening. Our hospice nurses are on-call 24 / 7 to support patients and their caregivers. If you or a loved-one has been diagnosed with a terminal illness (at any stage), please contact LifeCare to begin learning about hospice. LifeCare Palliative services is called AIMâ„¢ - Advanced Illness Management. AIM allows us to bridge the gap between home health and hospice. There is No prognosis requirement and No limitation on curative care. LifeCare's innovative AIMâ„¢ program bridges the gap between traditional home health and traditional hospice care. Thousands of families in East Texas are at home managing very advanced illnesses, and they need the type of expertise and service that hospice provides. However, a very large percentage of these people do not receive hospice services in a timely manner for two reasons. Medicare's hospice rules require that a patient have a prognosis of six months or less. Many diseases, especially non-cancer diagnoses, do not run a predictable course. Therefore, the doctor cannot always make the six-month prognosis that Medicare's rules require. Medicare's rules also require that hospice patients be done with treatments that are intended as curative. Often, patients who would benefit from palliative care services are not done exploring their curative options. For this very important group of patients, LifeCare has developed the innovative AIM program. Our AIM program uses your home health benefit (from Medicare or insurance) to deliver care from hospice-trained nurses, support from a medical social worker certified in psychiatric counseling, rehabilitative therapies, home health aide support, and other palliative care services at no cost to the family. With AIM, prognosis is not an admission criterion, and patients in our palliative care program are free to pursue any medical treatment they choose, including curative treatments. Home Health Services are also provided by LifeCare. Many of the services available to you in a hospital or nursing home are also available in the less-costly comfort of your own home. We send nurses, therapists, and home health aides to your home where our experienced team can help with a number of medical needs.
