We want to help you make well-informed decisions to meet your health care needs. To help you prepare for your visit, we have printable patient registration and medical history forms. We invite you to fill them out online at home, where you have access to the necessary information. Our office is located in Kirkland's Totem Lake area. We also have a satellite office location in Monroe, Washington, which is staffed by Dr. Erich Naumann, specialist in surgery of the maxillofacial region For a complete biography of Drs.
