By working with hospital case managers and community providers, Mid-Delta has been able to cut hospitalizations by one-third (33%) over a year's period of time for exacerbation of chronic medical conditions, such as Congestive Heart Failure (CHF), Diabetes and Hypertension-related illnesses.
