Colorado's Littleton Care and Rehabilitation Center is a nursing and convalescent home offering clients a comfortable, safe, and therapeutic stay. The center has an interdisciplinary care team to meet with the client and the family members and develop a care plan, which will cover areas such as nursing, nutrition, activities, therapy, and social service. The care team has professionals such as activity director, treating therapist, nursing representative, director of nursing, physician, and registered dietician. The center's discharge plan includes a Òroad mapÓ to assist patients as they return back home and start their duties. Its outpatient care plan offers therapy programs, wellness classes, ongoing training and education, and health screenings.
