  All women 55 and younger will require a urine pregnancy test prior to the surgery, except those who have undergone a total hysterectomy. You will need to provide photo identification, your insurance card(s), and payment of any copay or deductible that is due prior to the surgery. Payment may be in the form of cash or check, and all major credit cards are accepted.
