As a part of the Center for Human Development, the Connecticut Outreach program includes residential support services for adults. It serves individuals with severe and prolonged mental illnesses. The program also features medication management and monitoring, budgeting, socialization and employment assistance solutions. The Connecticut Outreach program includes mental health and substance abuse recovery services. In addition, it provides personal care solutions. The program s Crossover is a group home that serves individuals suffering from the disorders of mental illnesses. The Connecticut Outreach program also includes money management, employment and academic counseling services. It features the Community Transitional Living Program.
