Grace Plaza Nursing and Rehabilitation Center, founded in 1972 and located in Great Neck, N.Y., is a nursing and convalescent home that offers a wide range of health care and social services to the senior citizens throughout the state. The organization provides various services that include short- and long-term care; intensive and outreach services; joint replacement; pain and prosthetic management; and chronic wound and Alzheimer s disease treatment. The center offers other services, such as short-term rehabilitation, rehabilitation dialysis, hospice, home care, rehabilitation, internal medicine, neurology, obstetrics, occupational health and physiatrist services. It features a facility of more than 200 licensed beds. The home for elders is approved by the Joint Commission on Accreditation of Healthcare.
