- Emergency CareThe Transition Care Coordinator calls patients after any visit to the emergency department and after hospital discharge. The Transition Care Coordinator will check in with the patient to see how they are doing, ask if they have any questions and schedule any necessary follow up appointments.
- ColonoscopyColonoscopy: The colonoscopy is the best test for colon cancer screening. This procedure is done to look for abnormalities inside the colon and rectum. A colonscope (a thin, tube like instrument with a light and camera on the end) is inserted through the rectum and into the colon. During this procedure samples (biopsies) are taken of any polyps or abnormalities.

- ImmunizationsA wellness and focuses on preventive health care and screenings. During your wellness visit your provider will review your health and risk factors, discuss recommended health care screenings, labs and immunizations. Your recommendations are individualized based on your health and risk factors. Wellness visits are recommended for men and women, and are often done yearly. Your preventive care schedule may differ from the standard recommendations, talk with your provider about a schedule best for you based on your health.
- Eye ExamIt is also important you get an eye exam done yearly to help evaluate for vision loss or eye damage due to diabetes.
- Pregnancy
- Labor and DeliveryOur providers offer family planning, birth control, prenatal care, labor and delivery care for normal risk pregnancies. OB care providers include Dr. Steinke, Dr. Graybill, Dr. Meyer and Dr. Rojas-Bita, Dr. Stoecker. The first OB appointment, generally between 10 and 12 weeks, will include a complete medical history and a thorough physical, including a pelvic exam, breast exam, urine test, and blood work. An ultrasound will be ordered at an outside facility either before or after this visit.
- Cervical CancerPap smears are a screening test for cervical cancer and abnormalities. A pelvic exam is part of this visit and checks for other possible abnormalities and health problems, this includes checking the uterus and ovaries. Routine pap smears begin at the age of 21, the frequency of pap smears and pelvic exams is based on your age and risk factors. Visit with your health care provider regarding the recommended schedule and testing for you.
- Colon CancerColon cancer screening is recommended for all adults beginning at age 50 and continuing until age 75. Some individuals will continue to receive colon cancer screening after the age of 75 based on individual risk factors. There are several types of colon cancer screening...
- Prostate CancerThe current recommendations for prostate cancer screening are that you discuss your symptoms and risk factors with your provider to determine what evaluation is best for you. Prostate cancer screening may involve a rectal exam and a PSA (blood test checking the prostate).

Brain health is about caring for the brain and using the strengths of the brain to help keep the brain strong and active as you are aging. Brain health involves living a healthy lifestyle, which includes a healthy diet, healthy sleep, regular exercise, stress management, and staying mentally and socially active. Certain health problems can also affect the brain health by causing problems with the circulation and blood flow in the brain. These health problems include: high blood pressure, high cholesterol, heart disease and vascular disease, diabetes, and smoking. If you have these health conditions, we want to partner with you to help control them. Also, if you are smoking it is recommended you work on quitting smoking (CDC, 2018).

- Behavioral HealthThe Circle of Health was designed collectively with the University of Wisconsin and the Department of Veteran Affairs in order to provide a visual guidance to identify potential target areas as it relates to Whole Health. Our Behavioral Health Specialist will utilize this to help patients identify areas of concern or areas of desired improvement. The Circle of Health is where goal-setting towards behavioral and mental healthcare begins.

- UltrasoundThe heart muscle strength is often measured by an echocardiogram test, which is an ultrasound of the heart. This test measures the ejection fraction (EF). The ejection fraction is a measurement evaluating the heart muscle strength and squeezing ability. There are different types of heart failure. In one type of heart failure, the heart muscle strength is weak and cannot pump well. In this type of heart failure, the ejection fraction measurement is low which means the heart muscle is weak. In other types of heart failure, the heart muscle strength is normal but the muscle is thickened and stiff, which causes problems with the heart’s ability to squeeze and pump.
- RadiologyFamily Practice of Grand Island offers extensive laboratory and radiology services within the clinic. When necessary, the provider may utilize the pathologist and/or reference lab services of Physicians Laboratory in Omaha or Saint Francis Medical Center in Grand Island.
