The Adult Foster Care is a MassHealth funded program designed to provide individual care services to adult clients with medical diagnosis who require assistance/or supervision for their Daily Living Activities to maintain independent lifestyles in a home setting. Bay State AFC, LLC provides nursing services by a Registered Nurse and care management by a Social Worker with a periodic home visit to train, provide support, health education topics and monitor clients' health status and adjustment to the Adult Foster Care program. The care giver will receive training, support and supervision to assure the client's well being. The Participant will receive personal care services in a home-like setting that allows the participant to enjoy a private home atmosphere.

Bay State AFC is an amazing adult foster care agency with a staff that is caring and understanding. They are very professional and friendly; if you are taking care of a friend or relative and you need the extra income along with the professional support of a registered nurse and a social worker who will go out of his way to help you and if you want to be stress free and have the competent care of a staff that is there for you 24/7 I would recommend you give them a call. They also speak Spanish!!
