- Primary CarePlease bring your child’s insurance card to each visit and show it to the receptionist at check-in. If one of our doctors is not named as your child’s Primary Care Provider (PCP) with your insurance company, you must call your company before your child’s appointment to change your PCP. If it cannot be changed, as per your insurance plan, you will be asked to self pay at the time of the visit. We verify your insurance at the time of the visit, the list of participating insurance below is current to the best of our abilities. When new plans are added to the insurance company lists, they may not be represented below and particpation with that plan will be determined on a case by case basis at the time of your appointment. - Emergency CarePlease do not go to the emergency room if directed by these websites without first calling and speaking with a doctor, even if the office is closed. One of our doctors is always on call and can be reached by calling the office phone number and following the prompts to reach the doctor on call, who will call to give you advice.
- ImmunizationsYou will also be required to complete the NYS Department of Health COVID-19 Immunization Screening and Consent form. If you would like to minimize your time in the office, please print, complete, and bring this form with you when you come to get vaccinated. Each child vaccinated will need to have their own completed form.
