The Hospice of Dubuque Team combines the skills and compassion of varied professionals and volunteers to enhance end-of-life care. Included as a part of this care team are the patient, family caregivers and the patient's attending physician. Care is individualized to each patient and family, thus enabling them to have their preferences and goals honored. Services are focused upon promoting family control, patient comfort and quality and dignity at the end of life. Hospice of Dubuque is committed to giving the best possible care by providing: Expertise in pain and symptom management Home visits by registered nurses and medical social workers Staff on call 24-hours a day, seven days a week Hospice aides to assist with personal care needs Physician services for the medical direction of the patient's care, provided by either the patient's personal physician or the Hospice of Dubuque Medical Director Volunteer services for caregiver respite and support Spiritual and other counseling for the patient and family Physical, occupational and speech therapy Massage and music therapy Dietary counseling Pharmacy consultation Bereavement services Prescription medications related to the terminal diagnosis Medical supplies and equipment (eg, wheelchair, hospital bed, oxygen, etc) which are medically necessary for the management of the terminal illness.
