Family Medical Care for ages 13 and up Regular Visit $85 Includes the evaluation and management of up to two problems for one patient, and urine and pregnancy tests if they relate to the two problems. Add $10 for each additional problem. Any additional required lab work will incur an additional charge. No membership fees. Preventive Exams Female Physical with PAP $215 Includes lab testing for liver, kidney, and thyroid problems; cholesterol, sodium, potassium, calcium, and nutrition; urine test; a test for anemia; PAP. Female Physical without PAP $160 Includes lab testing for liver, kidney, and thyroid problems; cholesterol, sodium, potassium, calcium, and nutrition; urine test; a test for anemia. No PAP. Male Physical $215 Includes lab testing for liver, kidney, and thyroid problems; cholesterol, sodium, potassium, calcium, and nutrition; urine test; a test for anemia; PSA and prostate exam.
