Heartland Health Care Centers’ skilled nursing and rehabilitation centers offer post-acute services for those transitioning from hospital to home. This specialized quality care helps shorten or eliminate hospital stays, laying the groundwork for a successful discharge. Some of these services include: high-intensity physical, occupational and speech therapies, cardiac recovery, stroke and other neurological recovery care, skin and complex wound care, I.V. services, diabetes management, oncology care, and orthopedic and joint replacement recovery. With individualized programs and a variety of post-acute services Heartland offers rehab hospital caliber health care options to meet the unique needs of patients and ensure that they receive outcomes-oriented care that will speed their recovery and their return home.

My aunt was placed there for rehabilitation while pending a false investigation. During her stay, I witnessed diapers being placed on 3rd shift: she is mobile with someone monitoring her. As her neice, I would go in early that morning and I'll come back later that evening to get her ready for bed. Some of the nurses were prompt at giving her medicine on time, but others were late a they got her medication mixed up. They were giving her breathing treatment which she didn't need or never had had any problems. They have an excellent team of CNAs and a great therapist team. The staff in leadership positions have such a nasty attitude and poor communication skills with no respect towards family members. The Social Services dept have case workers with no care or compassion for the clients their: sitting in their offices. The Social Worker assistant was the only one who would come and visit with and talked to my aunt. When leaving in the evenings, I would see blinds open in patient's rooms & still open when I come in the mornings. The neighborhood is not safe and people walked that area constantly!
