Visiting Nurse Association Healthcare Partners of Ohio, located in Cleveland, is a community health care organization. A team of home care executives and a voluntary board of directors oversee the planning and delivery of care to thousands of families throughout northeast and central Ohio. The organization has an independent family of agencies, including VNA Mid-Ohio, Hospice and Palliative Care Partners of Ohio, VNA CarePlus and VNA HouseCalls. The organizations provide a full range of care to persons wherever they live, work or play. The health care system has a staff of more than 700 professionals, and it teams up with hospitals, physicians, community agencies, and insurance companies to deliver family centered health care to a 17-county area in Ohio.
