The Delmar Gardens Family of nursing centers and retirement communities has been providing outstanding care to our residents since 1965. Delmar Gardens Home Care was established to extend this quality care into our patients' homes in a professional and supportive manner. Our multi-disciplinary team of professionals develops a plan of care to meet the medical needs of our patients. Delmar Gardens Home Care is a Medicare certified and state-licensed agency offering skilled at-home intermittent care for patients. Highly qualified nurses, therapists, social workers, and home health aides work in harmony to address each patient's individual needs. Our staff is caring and concerned, well-trained and highly qualified. Each member of our team is carefully screened and fully insured. Services are offered 24 hours a day, 7 days a week. Delmar Gardens Home Care believes our wide range of services reaches one step beyond the expected, by helping our patients maintain good health and continued independence. We work closely with families and physicians to develop and implement a total home care program.

Services available from home care may include: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Worker, and Home Health Aide. The case manager in collaboration with you and your physician will develop a plan of care to meet your specific needs. These services are goal oriented and intermittent.
