Heritage Park Assisted Living located in Carbondale Colorado offers a unique and specialized environment with 28 private studio apartments and 2-room suites. Our services include 24-hour response system, three meals a day plus snacks, daily activities program, housekeeping, laundry services and transportation to medical appointments. Heritage Park Assisted Living is located on a campus that also includes Heritage Park Care Center, a skilled nursing and rehabilitation center. If needed, assisted living residents can access these services without having to leave the campus.Transitional Care Suites and Respite Stay Suites also available to provide transition/care between SNF stay and home or provide care while caregivers have travel plans.
