Supports for Community Living Program offers a range of community-based support services for Medicaid-eligible adults with intellectual disabilities. The program provides family and group home services. It serves the residents of Warren, Logan, Butler and Allen counties in Kentucky. The program offers support services to develop skills in the areas of daily living, social interaction, artistic expression and employment. It manages a staff of case managers who liaisons and advocates and provides individual assessment, monitoring and coordination services. The program additionally offers speech, occupational and physical therapies. Based in Bowling Green, Ky., Supports for Community Living Program is operated by LifeSkills.
