General Info
High Country Health Care System is the only local not for profit agency serving the high country for over 30 years. Services offered: Home Health, Transitions, and Hospice. High Country Home Health has a team of RNs, Physical Therapists, and CNAs offering assistance to those people who have a skilled nursing need and are homebound. Our nurses teach self-management skills to help you establish a sense of control over acute or chronic health issues. They teach the uses, doses, & side effects of your medications to prevent medication errors, and, provide other skills such as wound care, catheter & ostomy care, and IV infusion. Our therapists help you restore, adapt, or improve your physical performance that was altered through injury or illness. They teach effective pain management to improve movement and overall wellbeing. Our CNAs are available to assist with personal care during this recovery period. Once you meet the goals established by you and your physician, and can safely remain in your home, you are discharged from our services. High Country Transitions provides volunteers and case management services to individuals with a life-limiting illness. Anyone is eligible regardless of medical treatment or homebound status. This service is provided at no charge to the client and anyone can make a referral. Our volunteers can assist with your activities of daily living (light housekeeping, meal prep, running errands, etc.), be a friend or companion, and, give respite and bereavement support for caregivers. Transitions care can be provided in your home, hospital, nursing home, or assisted living facility. Many people in nursing homes or ALFs have no family nearby and appreciate a volunteer who will read aloud or sit and talk. Friendship is a great gift to the elderly who have lost their ability to care for themselves. Sitting and chatting about current events, gardening, or life experiences gives meaning and purpose to the client's life. High Country Hospice has a team of dedicated healthcare professionals including RNs, CNAs, social workers, chaplains, bereavement counselors, and trained volunteers. They provide the highest quality comfort care and support to assist you in maintaining your dignity while optimizing your quality of daily life. This team offers: care in the comfort of your own home, greater pain management, education for you and your family on the disease process, emotional and spiritual support, and bereavement support for you and your family and friends. Our volunteers offer individualized services based on the needs of you and your family.

High Country Home Health provides the highest quality nursing care in the comfort and privacy of your home. We promote healthy living and a safe environment by teaching you self management skills to help you maintain your health and strength. We create personalized healthcare goals working with you and your physician. We monitor your illness, medications, and progress to ensure you meet your health goals and improve your quality of life. High Country Transitions offers support and reassurance for those individuals who are experiencing a rapid decline in health. It is provided at no cost to any individual with a life limiting illness of one year or less, or who has a rapid decline in their health and don't qualify for Hospice care. Transitions care can be provided in your home, hospital, nursing home, or assisted living facility. This program provides trained Hospice volunteers and a case manager who will contact community resources to help meet the client's needs. Reach out to Transitions for comfort and support when your life feels chaotic and out of control. High Country Hospice offers many services to the patient who has 6 months or less to live and is seeking palliative care. Our holistic approach to care giving utilizes a team of healthcare professionals including: your primary care physician, RNs, social workers, C.N.A.s, bereavement counselors, and trained volunteers. We provide to you and your family an overall better end of life experience in the comfort of your home.
