- RadiologyYou can grant proxy access to family members by completing this form and returning it to the CMH Health Information Management (HIM) department. By granting proxy access, you are voluntarily allowing them complete access to the contents of your Patient Portal record which may include problem list, allergies, medications, lab and radiology results, and other clinical documentation. Proxy access will remain in effect until revoked in writing and submitted to our HIM department.
- ChemotherapyOur cancer center provides treatment, therapy, preventative measure and support services. Our chemotherapy treatments are given by specialized nurses trained in chemotherapy administration.

- General SurgeryAt CMH Surgical Services, our board certified surgeons have office hours to meet with patients for pre and post operative care. Outpatient surgeries are performed at our Outpatient Surgery Center (OSC) located in the Kerry and Synda Douglas Medical Center. Since our physician clinics and the OSC all use the same electronic medical record, healthcare providers have access to all pertinent health information.

- Sports MedicineVirtual visits are not for emergencies. If you are having a medical emergency, dial 911 or go to the nearest emergency room. With a virtual visit, CMH providers can diagnose and treat patients for COVID-19 related symptoms and also common medical conditions such as sore throat, sinus problems, stomach aches, nausea and vomiting, fever, skin problems, and sports injuries. Some medical conditions cannot be treated by virtual visit and may require an in-person visit with a provider. The virtual visit provider will let you know if you need a different visit type.

Prior to starting any type of therapy program, it is important to consider your needs and the requirements of your physician.
