My wife Ms. Prakash Prabhakar (DOB 08/15/1937) was adviced by the Physcian of Indian University Health Nephrology CL WH for urine test. The Urine was supplied to the Indiana University Hospital Lab. 1111, Ronald reagan Parkway, Avon In 46123 on March, 6, 2018. She had an appointment with the Specialist Physcian of the above Department on 03/27/2018 and during the examination of my wife by the specialist, it came to light that the urine report has not been received from the lab.. The Physician again ordered the testing of the urine. She gave the urine for test thereafter. We enquired from the person concerned from the lab. about the report of the urine supplied on 03/06/2018 and she confirmed to having received the urine but could not say anything about the test report. This is the second time that the report was not sent to the physcian and the facts are available in the computer system of the lab. which can be verified. An old lady of 81 years suffering from kidney problem, if in the absence of reports, some thing serious happens, who will be responsible. Is there any answer available from the Hospital, if so the same may be made available to us. I would also request the authorities of the Hospitel to see that such things do not happen.
