      VistaCare is one of the leading providers of hospice care services in the United States. The system offers more than 90 Medicare-certified programs to over 5,000 patients in nearly 25 U.S. states. It provides services to patients in hospitals, assisted living communities and nursing homes. VistaCare specializes in palliative care and pain and medication management. The system also offers support services to patients suffering from life-limiting illnesses and heart, lung, kidney and liver diseases. It serves the physical, spiritual and emotional needs of patients and their families. In addition, VistaCare provides counseling and bereavement and personal care services. Founded in 1995, the system maintains a staff of more than 2,700 employees. It additionally operates Prayer of Jabez Hospice in Houston and Lovelace Sandia Hospice in Albuquerque in New Mexico.
